Accord's™ case management services are offered in three different counties in the metro. Whether you're looking for specific services or advice and plans, our case managers are there to help you live more independently. 

We provide case management and support services for individuals who qualify under the Developmental Disabilities (DD) Waiver, Community Alternatives for Disables Individuals (CADI) Waiver, and the Brain Injury (BI) Waiver. Our case managers work with people to complete assessments, implement an individual service plan, arrange and monitor waiver eligible services, and consult and advocate on behalf of the individual. We also assist with accessing health care providers, educational resources, and social services.
