Accord’s™ case management services are offered in three different counties in the metro. Whether you’re looking for specific services or advice and plans, our case managers are there to help you live more independently.
We provide case management and support services to individuals who qualify under the Developmental Disabilities (DD) Waiver, Community Alternatives for Disabled Individuals (CADI) Waiver, and the Brain Injury (BI) Waiver.
