Accord’s Case Management services are offered in four different counties in the metro (Anoka, Dakota, Hennepin, and Washington). Whether you’re looking for specific services or advice and plans, our case managers are there to help you live more independently.

Supporting Individuals in Navigating Services and Resources.

We provide case management and support services to individuals who qualify under the Developmental Disabilities (DD) Waiver, Community Alternatives for Disabled Individuals (CADI) Waiver, and the Brain Injury (BI) Waiver. Our experienced team connects individuals to a wide range of home and community-based services, providing personalized case management and support every step of the way. Whether coordinating care, accessing essential resources, or navigating program options, we help those we serve live more independent, self-determined lives.

What We Offer

Our Case Management team is dedicated to helping individuals achieve their personal goals and live as independently as possible. We work with people enrolled in a variety of state programs, including:
